Job Description: The RN Patient Navigator in LVPG General, Bariatric and Trauma Surgery serves as a primary point of contact for patients, their family members, and referring physicians to access care for weight loss surgery or experiencing issues with reflux disease at LVHN. The purpose of this integral position is to decrease fragmentation of care and coordinate services, including medical and surgical. The patient navigator is to serve as a clinical liaison for the new and existing patients in the surgical weight loss program or with gastroesophageal reflux disease. The navigator assists in administrative functions and in the development and implementation of patient care protocols, clinical pathways, and in updating patient policies and procedures for care of bariatric surgery patients or patients with reflux. The navigator assesses and monitors the continuity of care and expedites and coordinates the delivery of services to facilitate a patient's progress throughout the continuum of care, from point of first interest, through the qualification period, the surgery process, post-op care, and post discharge, to achieve desired quality outcomes. The navigator will coordinate patient care collaboratively across all departments, disciplines and providers.

Minimum Requirements: Graduate of an accredited school of nursing. At least two years of experience as an RN. Must have an active PA RN license.

Preferred Qualifications: BSN desired. Weight management experience or experience with patients diagnosed with gastroesophageal reflux disease strongly desired.

Licensure and Certifications: Must have an active PA RN license.

Influenza vaccination is required for all colleagues with patient contact unless medically contraindicated or due to a bona fide religious belief or practice, and is strongly encouraged for all colleagues.
